To update your address and request a replacement card, follow these steps on ILostMyCard.com:

Select Your Certification State and Course:
Choose the state where you are certified and the course you completed. -
Enter Your Name and Date of Birth:
Provide your full name and date of birth to access your records. -
Order Your Replacement Card:
Complete your order using a credit or debit card. Once the order is placed, you’ll receive a temporary copy of your certification via email immediately. -
Receive Your Permanent Card:
Your permanent card will be mailed to your updated address and should arrive within 3–5 weeks.
Please note: It will arrive in a plain white envelope with no identifying markings.
