Complete, Sign, and Notarize the Form
The affidavit must be fully completed, signed, and notarized. -
Submit the Form
Mail the completed form to the address listed on the document.
Note: Replacement certificates are typically mailed within 3 to 4 weeks after your application is received.
Tip: Be sure to include your email address at the top of the form. This will allow you to receive a receipt and a digital copy of your card to use while you wait for the permanent version.
